While the initial damage increase is small, its true potential lies in the fact that Blood Revenge can stack up to 10 times, and is proc'd every time an ally is attacked (or when Pain Overture is used), resulting in one of the most powerful buffs in the game. The damage buff applies to all damage, whether it be an auto attack or a skill.

Pain Overture can be used to maximize the stack of Blood Revenge - its cooldown of 8 seconds is fast enough to reactivate before Blood Revenge resets.

(Personal note, original page creator Kiljaedenas): I honestly have no idea how the damage boost for this skill is calculated because it doesn't follow any conventional math that I can think of. The damage boost is actually LARGER than any way I can calculate. I don't know if I got a strange version on my phone or what, but at level 4 it states the damage boost as 5.7% per stack when it in fact seems to be double that, and it adds up in an odd way.

Case in point: Around level 70 or so with Blade of Darkness maxed but no other damage buffs on, a regular attack shot does 402 damage to a particular enemy. Now if the 5.7% boost of Blood Revenge at level 4 just added on top of each other that would lead to a 57% boost, which would mean I would be hitting for 631 with all 10 stacks. If it multiplied on top of itself, that would lead to a 74.1% boost with all 10 stacks on, meaning I would hit for 700 damage. But with all 10 stacks I actually end up hitting for 909 damage which is a 126% boost. Not that I'm complaining, but I just don't get the math.

Theory by Ghostly Owns: Getting similar result, and I have a theory. The Blood Revenge buff growth is not linear, but exponential: instead of damage x (1.062 x # of stacks), it's damage x (1.062)^{# of stacks}. I'd do the math, but I'm too lazy to do it.
